Key Responsibilities:

Complete comprehensive home health assessments, averaging 45 minutes per visit, for Medicare Advantage members Review and evaluate medical, surgical, and medication histories during home visits Educate patients about their health conditions, medications, and preventative measures Identify potential gaps in care and provide recommendations to ensure optimal patient outcomes No need for treatment plan development or prescribing of medications

Qualifications:

Current Nurse Practitioner license in Wisconsin Board Certification through AANP or ANCC Active CPR certification (BLS) MSN degree from an accredited nursing program At least 6 months of NP experience preferred Experience with home health or risk assessment visits is a plus
