Great Lakes Home Health Care is looking for an experienced RN to join our growing team. We are passionate about giving the very best care to improve patient lives and outcomes. The Home Health Registered Nurse (RN) provides and directs provisions of nursing care to clients in their homes in collaboration with the primary health care provider, and in compliance with applicable laws, regulations, and agency policies. Also, Registered Nurse (RN) coordinates the plan of care in conjunction with other health care professionals. The home health RN helps to achieve excellent clinical outcomes of care through the delivery of nursing process.
Position:
Home Health Registered Nurse (RN) - Full time License Requirements Must have RN license for the state of Wisconsin Active driver's license and valid insurance
Qualifications:
Work independently, make quick decisions, and adapt to changing patients' needs Provide care to patients as outlined by the Plan of Care Experienced in nursing assessments, wound care, and medication management
