Develop care plans including discharge planning, coordinate delivery among providers, and teach patients and families how to follow the plan. They may also help patients navigate insurance and other healthcare bureaucracies and access related social services. May work with patients who have chronic illnesses or other complex medical needs. Works at a hospital or other healthcare facility; may follow patients throughout the span of their treatment.

Hospice RN Case Manager Location:

Milwaukee County Pay:

Up to $45/hr and a $10,000 sign on bonus Want meaningful work without giving up your life outside of work? 15-18 patient caseload average 5-6 visits/day Monday-Friday 40 hours No routine on-call or weekends. Limited to vacation and sick coverage. Dedicated after-hours on-call team Company vehicle provided Milwaukee County only Collaborative, supportive IDG Hospice Experience not required.
