Key Responsibilities:

Perform comprehensive annual and post-discharge home health risk assessmentseach visit averaging around 45 minutes. Review and document prior and current medical and medication histories. Educate patients and their families about health conditions, wellness, and ongoing medication management. Identify care gaps and offer practical recommendations for improved health outcomes. No prescribing or creation of treatment plans required; focus is on evaluation, education, and closing care gaps. You'll travel within Shawano County (including Shawano, Bonduel, and Birnamwood), conducting scheduled visits in various communities to support patients in their own homes.

Qualifications:

Active Nurse Practitioner license in Wisconsin Current CPR (BLS) certification Board certification through AANP or ANCC Minimum of 6 months recent nurse practitioner experience preferred Master of Science in Nursing (MSN) from an accredited program Benefit from competitive per-evaluation compensation of $80$90, along with mileage reimbursement and additional support for administrative documentation.
