- Registered Nurse MDS Coordinator (RN/LPN) is responsible for:
- Coordinating the RAI/MDS process, ensuring accurate and timely completion of assessments and care plans
- Collaborating with the interdisciplinary team to identify and address resident needs, goals, and preferences
- Developing and implementing individualized care plans, incorporating resident-centered care and evidence-based practices
- Maintaining accurate and up-to-date resident records, including care plans, progress notes, and incident reports
- Participating in quality improvement initiatives, identifying areas for improvement, and contributing to the development of solutions
- Providing education and support to nursing staff, family members, and facility staff on RAI/MDS processes and best practices

Requirements:
- Current RN or LPN licensure in the state of Wisconsin
