Your expertise in nursing care and RAI/MDS practices will enable you to effectively assess, plan, and document resident care, while collaborating with our interdisciplinary team to deliver high-quality care.

  • Coordinating the RAI/MDS process, ensuring accurate and timely completion of assessments and care plans

  • Maintaining accurate and up-to-date resident records, including care plans, progress notes, and incident reports

Requirements:
  • Current RN or LPN licensure in the state of Wisconsin
  • One year of experience in long-term care, acute care, or rehabilitation setting (or equivalent education and training)
  • Strong knowledge of RAI/MDS regulations, guidelines, and best practices
  • Ability to work effectively in a fast-paced environment, with strong communication and problem-solving skills
