  • Full-time, Contract Job Overview We are seeking a dedicated Hospice Registered Nurse Case Manager to join our compassionate healthcare team.
In this vital role, you will coordinate and oversee comprehensive end-of-life care for patients in a hospice setting, ensuring that each individual receives personalized, high-quality support aligned with their needs and wishes. The ideal candidate will possess extensive clinical expertise, including critical care, ICU, and emergency medicine experience, to effectively manage complex cases and facilitate seamless care transitions. This position offers an opportunity to make a meaningful difference in patients' lives while working within a collaborative, patient-centered environment.
Shift:
12hr Day Shift; Every weekend (Saturday and Sunday) 8a-8p

Requirements:
Must have WI/Compact State RN license Must have 2 years of Home Health or Hospice experience. Must have BLS from
AHA. CA02

$3,000.00
