• Hospital & Community Coordination Key Responsibilities Provide hospice liaison services for referrals from hospitals and skilled nursing facilities Conduct patient assessments and determine appropriate hospice level of care Admit patients to hospice services in hospital settings when appropriate Collaborate with physicians, hospital staff, and interdisciplinary care teams Educate patients and families on hospice philosophy, services, and benefits Manage symptom support discussions (pain, dyspnea, end-of-life care needs) Utilize

for documentation and communication Support transitions of care across multiple hospital systems and facilities Requirements Active RN License (WI or compact eligible) Minimum 2 years of nursing experience required (hospice experience preferred)
