Job Description

The Registered Nurse (RN) Care Coordinator is responsible for delivering comprehensive, tenant-centered care through the application of the nursing process, including assessment, planning, intervention, evaluation, and documentation, in accordance with Tamarack Health's policies and standards. The RN Care Coordinator works collaboratively with physicians, multidisciplinary teams, and other healthcare professionals, ensuring the highest level of care is provided to tenants. This role requires independent decision-making, professional judgment, and the ability to delegate patient care based on licensure and expertise. The RN Care Coordinator will work closely with all members of the RCAC team to foster a positive work environment and promote staff professional growth.
Responsibilities/Duties:
Patient-Centered Care
  • Utilize the nursing process to provide safe, effective, and individualized care to patients, residents, or tenants without constant supervision. Assess patient needs, prioritize care, and implement interventions based on current conditions. Comprehensive Assessments
  • Conduct thorough assessments of tenants, prioritizing data collection based on their immediate needs. Follow established timeframes for documentation and care delivery in alignment with facility policies and protocols. Care Plan Development & Collaboration
  • Collaborate with interdisciplinary team members to develop, implement, and revise individualized care plans. Ensure care plans reflect patient preferences, including cultural, ethnic, religious, and spiritual needs, along with evidence-based nursing practices. Treatment and Documentation
  • Administer prescribed treatments accurately and in a timely manner. Document assessments, interventions, and responses to care in the medical record, ensuring thorough, up-to-date, and accurate information. Emergency Response
  • I nitiate emergency resuscitative measures according to established protocols, ensuring patient safety in critical situations. Confidentiality & Communication
  • Maintain confidentiality at all times, respecting the privacy of patients, families, and staff. Utilize SBAR (Situation, Background, Assessment, Recommendation) for effective communication during shift changes. 24-Hour Report Management
  • Take ownership of the 24-hour report, ensuring it is initiated, updated, and completed consistently, emphasizing accurate communication and follow-through across shifts. Ongoing Leadership & Support
  • Keep the Campus Administrator and RCAC Supervisor informed about situations that may impact patient care or the organization, including potential issues identified during daily operations or surveys.
Education/Experience:
Required Current Wisconsin Registered Nurse (RN) license Associate's Degree in Nursing Basic Life Support (BLS) certification, required prior to providing direct patient care
