Key Responsibilities:

Perform comprehensive annual home health risk assessments for Medicare Advantage members in their homes Review and evaluate patients past and current medical history, existing conditions, and medication regimens Educate patients on their health conditions and prescribed medications Identify care gaps and provide clinically sound recommendations to encourage wellness and reduce risks Visit locations throughout St. Croix County as assignedno treatment planning or prescribing required

Qualifications:

Active Nurse Practitioner License in Wisconsin Current CPR certification (BLS) Board Certification through AANP or ANCC required

Preferred:
