About the
Role:
The Hospice Home Team Weekend RN plays a critical role in delivering compassionate, patient-centered care to individuals facing life-limiting illnesses during weekend shifts.
Hours:
Every Other Saturday/Sunday from 8am-8pm
Location:
Territory will include Sheboygan and Manitowoc Counties, with the potential of Ozaukee County as well. This position ensures continuity of care by managing symptom control, providing emotional support to patients and their families, and coordinating with interdisciplinary teams to uphold the highest standards of hospice care. The RN will assess patient needs, implement care plans, and respond promptly to changes in patient conditions to enhance comfort and quality of life. This role requires a strong commitment to holistic care, respecting patient dignity and preferences while navigating complex clinical situations. Ultimately, the Hospice Home Team Weekend RN contributes significantly to the mission of providing empathetic, expert care during some of the most vulnerable moments in patients' lives.
Responsibilities:
Conduct comprehensive patient assessments to evaluate physical, emotional, and psychosocial needs during weekend shifts. Administer medications and treatments as prescribed, ensuring adherence to hospice care protocols and safety standards. Collaborate with interdisciplinary team members including physicians, social workers, and chaplains to develop and update individualized care plans. Provide education and support to patients and their families regarding disease progression, symptom management, and available resources. Document all patient interactions, care provided, and changes in condition accurately and timely in the electronic health record system. Respond promptly to patient and family concerns or emergencies, coordinating with on-call providers as necessary. Maintain compliance with regulatory requirements and organizational policies related to hospice nursing care.
Minimum Qualifications:
Current and valid Registered Nurse (RN) license in the state of practice. Minimum of two years of clinical nursing experience, preferably in hospice, palliative or home care, or a related healthcare setting. Basic Life Support (BLS) certification. Demonstrated ability to perform comprehensive patient assessments and develop nursing care plans. Strong communication skills to effectively interact with patients, families, and interdisciplinary team members. Certification in Hospice and Palliative Nursing (CHPN) within 3 years of hire.
Skills:
Utilize clinical nursing skills daily to assess patient conditions, administer medications, and implement care plans tailored to individual needs. Strong interpersonal and communication skills are essential for providing emotional support and education to patients and families, fostering trust and understanding during difficult times. Critical thinking and problem-solving abilities enable the nurse to respond effectively to changes in patient status and coordinate care with the interdisciplinary team. Organizational skills are employed to maintain accurate documentation and ensure compliance with healthcare regulations. Additionally, proficiency with electronic health record systems facilitates efficient information management and continuity of care across weekend shifts.
