RN Care Coordinator Felician Village |
Long-Term Care Schedule:
11:00 AM - 7:00 PM | Full-Time Felician Village is seeking a compassionate and experienced Registered Nurse (RN) Care Coordinator to join our long-term care team. This position provides a combination of direct resident care, care coordination, admissions, medication administration, and clinical support for the nursing team. The ideal candidate is a dependable RN who enjoys working closely with residents, families, CNAs, nurses, providers, and the interdisciplinary team to provide safe, person-centered care. Responsibilities Provide compassionate, hands-on nursing care to long-term care residents. Complete new resident admissions, including nursing assessments, medication reconciliation, and initial care planning. Perform ongoing assessments and monitor residents for changes in condition. Administer medications and treatments safely and accurately according to provider orders and facility policies. Develop, implement, and update individualized resident care plans. Coordinate resident care with CNAs, LPNs, RNs, physicians, therapists, social services, dietary staff, and other members of the interdisciplinary team. Communicate changes in resident condition to providers and follow up on orders and recommendations. Provide education and support to residents and their families regarding care, medications, treatments, and changes in condition. Participate in care conferences and contribute to the development and evaluation of resident-centered care plans. Assist with transfers, hospital returns, appointments, and changes in level of care. Maintain accurate and timely clinical documentation and ensure required nursing documentation is completed. Provide clinical guidance and support to nursing staff and CNAs. Promote resident dignity, safety, comfort, and quality of life. Follow infection prevention, medication management, fall prevention, and other long-term care standards. Respond appropriately to emergencies and changes in resident condition. Support continuity of care and effective communication between shifts. Participate in quality improvement activities and other nursing responsibilities as assigned. Qualifications Current, active Registered Nurse (RN) license in the State of Wisconsin. Previous long-term care, skilled nursing, rehabilitation, or geriatric nursing experience preferred. Experience with resident admissions, medication administration, assessments, care planning, and documentation preferred. Strong clinical assessment and critical-thinking skills. Excellent communication and interpersonal skills. Ability to work collaboratively with residents, families, and an interdisciplinary healthcare team.
