Capuchin Franciscan Province of St. Joseph The Capuchin Franciscan Province of St. Joseph is a religious community of friars inspired by St. Francis of Assisi. The friars live and work with those who struggle with challenging circumstances and serve God through prayer and the work they do, in the communities they serve. Position Overview We are seeking a full-time Health Care Coordinator to join our team. The Health Care Coordinator, based in Wisconsin and under the direction of the Director of Clinical Operations, will manage and coordinate the health care for friars in Mt Calvary, Milwaukee, Chicago, and as needed in Montana. This position will serve as a resource to the guardians of these Capuchin communities as well as the Director of Clinical Operations. Essential Duties and Responsibilities Demonstrate, exemplify and support the Capuchin Charism and Provincial Mission, Vision and Values throughout all professional responsibilities and activities. Performs duties following state Statutes for Nursing Standards of Practices and Code of Conduct defining scope of practice by licensed nurses. Develop plan of care for as needed for friars with acute/or chronic health conditions. Such plans will be reviewed with the Clinical Operations Director and guardian as needed and documented as required. Assist friars in managing their health problems, provide disease-specific education; provide guidance when specialty care is necessary providing referrals to doctors and community services. Create and maintain a 6-month check-in system for friars with chronic but stable conditions and a more frequent check-in for friars who need more care or whose conditions are not stable, documenting as needed. Be available and follow up with friar concerns to demonstrate and foster an environment of health care support focusing on overall wellbeing. Be available and promptly respond to all communication from friars, supervisor, and guardians on a regular basis throughout the day. Attend and participate in workshops and conferences to develop and maintain current knowledge of wellness, geriatrics, retirement and insurance issues. Actively participate in discharge planning and aftercare for friars who have been hospitalized. Work with discharge planners, social workers, guardians and others to manage care and respect friars' choices. Assist in coordination of care for friars transitioning from healthcare locations and levels of care ensuring friars, guardians and Clinical Operations Director are all kept informed. Develop and maintain up to date information of various housing and level of care options for each general location served, proactively developing rapport with healthcare staff of clinics, hospitals, nursing homes and other living facilities to facilitate communication when the need arises. Maintain medical records and keep current Emergency Medical Records on all friars, making timely, meaningful nursing note entries after incidents, appointments, hospitalizations, and clinical conversations. Track and assist in managing Power of Attorney records on all friars. Maintain a reliable system that tracks current medical appointments, procedures and results for all friars. Facilitate and/or accompany friars to medical appointments when needed or when the complexity of medical information requires a nurse present for better understanding. Assist in the implementation of physician orders and follow up within a reasonable time frame; evaluate for compliance status and appropriate outcomes. Communicate timely to the Clinical Operations Director when orders are not being followed, care is not being delivered, plans of care are not being managed, and/or outcomes are unfavorable. Submit weekly reports to Clinical operations Director each Friday. Comply with Province and ministry policies, procedures, guidelines and standards. Qualifications Current RN license required. Minimum of five (5) years of experience as an RN or LPN with progressive responsibilities, including leadership experience. Minimum of two (2) years of geriatric nursing experience required; skilled nursing and/or long-term care experience preferred. Minimum of two (2) years of experience in a nurse supervisor role or as a Minimum Data Set (MDS) nurse. Effective Microsoft Office skills. Effective communication, problem solving, interpersonal and organizational skills. Strong health assessment skills. Ability to remain available by phone during non-business hours to provide guidance for emergencies. Ability to maintain confidential information and preserve the dignity of friars. Ability to be engaging, welcoming and professional. Ability to travel to Milwaukee, Mt. Calvary, Appleton, and Chicago. Ability to lift and move up to 25 pounds.
