Join Preceptor Home Health & Hospice, a trusted provider dedicated to delivering high-quality, patient-centered care. As an RN Case Manager, you will play a vital role in coordinating care, supporting patients and families, and helping individuals thrive in the comfort of their homes. What You'll Do As an RN Case Manager, you will oversee and coordinate patient care while working closely with an interdisciplinary team.
Responsibilities include:
Developing, implementing, and updating individualized plans of care Performing comprehensive patient assessments and ongoing evaluations Providing skilled nursing care and carrying out physician-ordered treatments Educating and counseling patients, families, and caregivers Coordinating care with physicians and other healthcare professionals Managing documentation, with a majority completed in the patient's home Leading and participating in case conferences and care planning discussions Identifying and implementing discharge planning needs Communicating patient status updates to physicians and care team members Supervising and evaluating care provided by LPNs and Home Health Aides Ensuring necessary equipment and services are arranged for patients What We're Looking For Current Registered Nurse (RN) license in the State of Wisconsin Graduate of an accredited nursing program Home health or public health experience preferred (not required) Strong clinical assessment and critical thinking skills Excellent communication and organizational abilities Ability to work independently and manage a caseload effectively Valid driver's license with a reliable, insured vehicle Physical ability to perform patient care tasks (lifting, standing, mobility support, etc.) Why Join Us?
