Job Description

As an MS Stroke Stepdown Nurse at Trusted Health, you'll provide specialized, evidence-based care to adult stroke patients transitioning from ICU-level care. Working in a fast-paced stepdown environment, you'll conduct focused neuro assessments, monitor telemetry, manage IV medications, and rapidly respond to changes in patient status. You'll collaborate closely with physicians, therapists, and case managers to create and execute individualized care plans that support optimal recovery and safe discharge. In this mission-driven, nurse-centric culture, you'll leverage technology-enabled tools, clinical support resources, and data-driven protocols to streamline workflows and enhance patient outcomes. You'll also educate patients and families on stroke prevention, symptom recognition, medications, and lifestyle modifications, playing a key role in reducing readmissions and improving long-term quality of life. This role is ideal for clinically strong RNs who are passionate about neurology, stepdown care, and shaping a more modern, supportive experience for both clinicians and patients. Responsibilities Provide comprehensive MS stroke stepdown nursing care to a complex adult patient population Continuously assess neurological status and escalate changes promptly Administer medications, manage IVs, and monitor telemetry and vital signs Collaborate with physicians, therapists, and the care team to develop and implement care plans Educate patients and families on stroke recovery, medications, and discharge planning Document assessments, interventions, and outcomes accurately in the EHRSupport quality and safety initiatives, including stroke protocols and core measures Precept and mentor less experienced nurses as needed Required Skills Stroke assessment and management Neurological assessments Telemetry monitoring IV medication administration Care of MS/stepdown patients Care planning and coordination Electronic health records (EHR) documentation Acute care nursing procedures Patient and family education Interdisciplinary team collaboration
