A Nurse Case Manager applies nursing expertise to coordinate a patient's medical care and provides advocacy to help them get the best and most affordable care. Develop care plans including discharge planning, coordinate delivery among providers, and teach patients and families how to follow the plan. They may also help patients navigate insurance and other healthcare bureaucracies and access related social services. May work with patients who have chronic illnesses or other complex medical needs. Works at a hospital or other healthcare facility; may follow patients throughout the span of their treatment.

Job Description
Position Overview:
RNCM would be responsible for providing skilled and compassionate nursing care and support to patients and families where they call home. Implements thorough skilled nursing assessments to include physical, psychological, social, spiritual, and environmental statuses, identify patient goals and appropriate interventions, and create an individualized plan of care for each patient. Performs skilled nursing care in accordance with standards of practice and per Castle Hospice policies and procedures and the patient's plan of care. Continuously reviews the plan of care and revises the plan of care in accordance with the fluidity of patient and family needs. RNCM would coordinating schedule with the Hospice Director of Nursing, or designee and would involve an on-call rotation.Qualifications:
Current certification as a Registered Nurse with the state of Wisconsin. Exceptional communication, problem solving skills, and have a compassionate bedside manner. Maintains current automobile insurance and valid driver's license.
