We are a continuing care campus offering an array of services and settings including independent apartments, CBRF, RCAC, and a skilled nursing facility. Compassionate care is delivered in a faith-based environment, emphasizing independence, respect, and dignity. We currently have an opening for a Full-Time experienced MDS Coordinator (RN) . The role of MDS Coordinator is unique and requires someone with a minimum of one year of experience in care planning, assessment and the entire MDS process. Critical thinking, leadership skills, flexibility, excellent communication skills, and a focus on team building is a must. Essential Functions of the

Job:

• Working knowledge of Medicare rules and procedures, with expertise in Medicare documentation.
• Proficiency in computer data entry and retrieval.
• Knowledge of nursing principles (geriatrics, long-term care, rehabilitation, and acute medical care) and professional standards of nursing practice, and ability to apply to residents' specific circumstances. Able to identify, implement, and evaluate appropriate objectives and interventions for residents.
• Demonstrated competency in clinical skills needed for facility resident population.
• Demonstrated competency in physical, behavioral and functional capacity assessment of long-term care residents.
• Working knowledge of laws and regulations that influence provision of care and services in nursing facilities.
• Working knowledge and ability to interpret and demonstrate facility policies, procedures to direct care staff.
• Ability to teach small groups of nursing staff in classroom and on unit.
• Sufficient organizational and interpersonal skills to assign, motivate staff, elicit work output, improve quality and interact effectively with residents, families, interdisciplinary team members, administration, government officials, consultants and others in addition to staff.
• Carries out all duties in accordance with the facility mission and philosophy.
• Demonstrates knowledge of medications and their correct administration based on age of the patient/resident and their clinical condition. Evaluates patient/resident responses to medications.
• Observant for significant changes in resident's condition and communicates this to the physician and IDT for the purpose of the MDS.
• Demonstrates and is knowledgeable of the importance of maintaining confidentiality of resident and facility information.
• Plans for and provides patient/resident, family and caregiver education, identifies learning needs, clearly communicates information and documents patient/resident response to education.
• Able to understand and follow written and verbal directions. Able to express self adequately and accurately in written and/or oral communication (including documentation in clinical records), with direct care staff, interdisciplinary team members, administration, and government officials.
• Must have organizational skills, ability to assess, plan and implement and evaluate patient/resident care. Must be able to demonstrate ability to handle multiple priorities and the ability to direct and evaluate the activities of others.
• Ability to oversee and effectively guide the restorative program staffers.
• Sufficient mobility and strength to move freely through the building to assure resident safety at all times and to assist, transfer, or otherwise move residents of facility out of danger in case of emergency. Job Requirements
• Must be a graduate of an accredited School of Nursing.
• Must possess a current, unencumbered license to practice as a RN in the State of WI.
• Minimum one year of experience as a MDS Coordinator preferred.
• Must possess current CPR Certificate or ability to obtain certification within 30 days of hire.
• Must project a positive image of the facility by displaying a high degree of professionalism, demonstrating concern and courtesy in interactions with residents, families, medical staff, employees, volunteers and outside agencies affiliated with the facility.
• Must demonstrate the ability to function independently, have personal integrity, flexibility, and the ability to work effectively with residents, personnel and support agencies, and to make independent decisions when necessary.
• Must be able to communicate effectively and clearly with the Director of Nursing and all coworkers, staff, residents, and families on an ongoing basis, utilizing tact, clarity, discretion and fairness.

Schedule:

Primarily day shift, but required to maintain a work schedule that ensures the presence of the MDS RN on periodic weekends or evenings, nights and holidays. The expectation is that the MDS RN work 40 - 45 hours per week and to participate in a call rotation one week per month (may require occasional presence in the building during call week) among nursing leadership to ensure nursing department support as needed.
