An opportunity is available for an experienced Nurse Practitioner with a background in home health to perform in-depth risk assessments for Medicare Advantage members across St. Croix County. Flexible shifts available Monday through Sunday Set your own schedule Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

Perform thorough home health risk assessments, each lasting approximately 45 minutes Review patients medical histories, current conditions, and medication regimens during home visits Document findings and provide clear education to members about their health and treatment Identify and communicate potential gaps in medical care with recommendations for resolution Typical visits may include annual assessments or post-discharge evaluations Locations may include Hudson, River Falls, and New Richmond based on shift assignment Please note: This role does not require developing treatment plans or prescribing medications

Qualifications:

Active Nurse Practitioner license in Wisconsin MSN degree from an accredited school of nursing Board Certification via AANP or ANCC Current CPR certification (BLS) At least 6 months experience as a Nurse Practitioner strongly preferred Direct experience in home health a plus This per diem position offers $80$90 per completed evaluation as well as stipends for mileage and administrative coordination.
