Clinic Registered Nurse Primary Care Clinic Full Time / 80 hours per pay period 8 or 10 hr. shifts / 4 or 5 days per week Come join our team! Excellence Essential Job Functions Provides individualized nursing care across the lifespan in an ambulatory setting using the professional nursing process, including focused assessment and clinical decision-making. Collects and interprets patient data to identify needs and support appropriate, patient-centered care planning. Delivers care across the continuum, including prevention, health maintenance, diagnosis, treatment, and follow-up. Coordinates care within the RN scope of practice, including triage, screenings, patient education, and collaboration with providers and the care team. With the appropriate training, may assist with providing comprehensive care coordination activities, including Chronic Care Management (CCM), Transitional Care Management (TCM), Principal Care Management (PCM), INR management and the American Diabetes Association- Certified Diabetes Education Program .
