Schedule Overview:

Primary Responsibilities:

Complete annual and post-discharge home health risk assessments for plan members in the region, including Hudson, River Falls, and New Richmond Assess patients medical history, current health status, chronic conditions, and medication regimens during 45-minute visits Deliver patient education related to health conditions and prescribed medicines Identify unaddressed health needs, offering recommendations to close care gaps No responsibility for treatment plan creation or prescribing medication

Qualifications:

Active Nurse Practitioner license in Wisconsin Board Certified through AANP or ANCC Graduate degree (MSN) from an accredited nursing program Current BLS/CPR certification At least 6 months of recent NP experience preferred
