Key Responsibilities:

Complete annual home health risk assessments for Medicare Advantage members Review and evaluate patients medical histories, current conditions, and medication regimens Educate patients on their health conditions and treatment plans Assess for gaps in care and offer actionable recommendations to improve health outcomes No treatment plan creation or prescribing requiredfocus is on comprehensive assessment and education

Qualifications:

Active Nurse Practitioner license in Wisconsin Current CPR certification (BLS) Board Certification through AANP or

ANCC MSN

degree from an accredited nursing program At least six months of NP experience preferred
