Key Responsibilities:

Conduct annual and post-discharge home health risk assessments for Medicare Advantage plan members Evaluate each patients prior and current medical history, medical conditions, and medication use Educate patients on their health conditions and prescribed medications Identify care gaps and provide recommendations for care coordination Travel throughout Hudson, River Falls, New Richmond, and other locations within St. Croix County for home visits No need to develop ongoing treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner (NP) license in Wisconsin Current CPR certification (BLS) Board Certification through AANP or

ANCC MSN
