Responsibilities:

Perform annual and post-discharge home health risk assessments for Medicare Advantage plan members Review and document prior and current medical history, medical conditions, and medications Educate patients on their health conditions and prescribed medications Identify gaps in care and offer actionable recommendations to support optimal patient health No responsibility for creating treatment plans or prescribing medications

Qualifications:
