Responsibilities:

Conduct annual home health risk assessments for members of a Medicare Advantage plan Assess and document prior and current medical histories, conditions, and medications Offer patient education regarding medical conditions and medication management Identify gaps in care and provide recommendations to address them Adhere strictly to evaluation, education, and documentation; prescribing and treatment planning are not required Each evaluation averages 45 minutes, including administrative time and travel across various communities within St. Croix County (e.g., Hudson, River Falls, and New Richmond)

Qualifications:

Active Nurse Practitioner license in Wisconsin Current Basic Life Support (CPR) certification Board Certified (AANP or ANCC) Master of Science in Nursing (MSN) from an accredited institution At least six months of current experience as an NP is preferred Youll benefit from competitive compensation, earning $80$90 per completed evaluation along with a mileage stipend and administrative time included.
