Hospice Executive Director Position Summary St. Camillus Hospice is seeking a compassionate and skilled Nurse Practitioner (NP) to join our dedicated hospice care team. This role is responsible for conducting face-to-face assessments for hospice patients under the direction of the Hospice Medical Director. The Nurse Practitioner will utilize clinical expertise and sound judgment to evaluate patient eligibility for hospice services and collaborate with interdisciplinary teams to ensure high-quality, patient-centered care. Key Responsibilities Perform face-to-face assessments for patients entering their 3rd and subsequent hospice certification periods Conduct additional patient evaluations at the direction of the Hospice Medical Director Assess patient eligibility and ongoing appropriateness for hospice services Collaborate with the Medical Director and interdisciplinary team to develop and coordinate care plans Participate in Interdisciplinary Group (IDG) meetings and care conferences Provide detailed documentation of home visits, phone calls, and clinical findings in accordance with regulatory standards Maintain accurate and compliant clinical records following agency policies and state/federal regulations Communicate patient condition updates to the Medical Director Participate in quality assurance initiatives and ongoing performance improvement Ensure confidentiality of patient and staff information Promote hospice services within the community Adhere to all organizational policies, safety guidelines, and the St. Camillus Code of Conduct Qualifications Master's Degree in Nursing from an accredited program Current Wisconsin Nurse Practitioner license required CPR certification required Minimum 1 year of Nurse Practitioner experience preferred 2+ years of hospice experience strongly preferred Required Skills & Competencies Excellent communication and interpersonal skills Ability to provide compassionate communication regarding end-of-life care to patients and families Strong clinical assessment and decision-making skills Ability to work independently and collaboratively within an interdisciplinary team High level of accuracy and attention to detail in documentation Adaptability to different patient personalities and needs Requirements Ability to safely perform essential job functions with or without reasonable accommodation Valid driver's license, proof of current auto insurance, and a driving record that meets St. Camillus Safe Driving Policy Why Join St. Camillus Hospice?
