Wellness and Care Management Nurse St. Camillus - 3.2 Wauwatosa, WI Job Details Full-time 11 hours ago Benefits Wellness program Qualifications RN License Driver's License Full Job Description Nursing, the way you always hoped it would feel Remember why you became a nurse? It probably wasn't for call lights, double shifts, and charting at a sprint. At St. Camillus, our Wellness & Care Management Nurse gets to practice the kind of nursing that made you fall in love with the profession: getting to know people, helping them thrive, and seeing the difference you make every single day. You'll be the go-to health partner for our Independent Living residents, a vibrant community of older adults with stories, wisdom, and a great sense of humor. Some days that means a blood pressure check and a good conversation. Other days it means building a care plan that helps someone stay independent and in the home they love. Why you'll love it here Trade the hospital chaos for real connection. No overhead pages, no revolving-door patient census. You'll care for the same residents over months and years, and they'll know you by name (and probably save you a cookie). Prevention, not just crisis. Your focus is keeping people well, active, and independent. You'll catch small changes before they become big problems. Room to think and plan. You'll have the time to actually listen, educate, and build thoughtful care plans instead of racing from task to task. Your ideas shape the program. You'll help grow our Revitalize Wellness program, from fitness classes to health talks to partnerships with local universities and community groups. Work that fills your cup. Older adults have so much to teach. Helping someone stay independent, reassuring a worried family member across the country, or simply making someone's day brighter is the kind of reward that sticks with you. A mission you can feel. St. Camillus is built on caring for the whole person: physical, social, and emotional. Here, that's not a slogan; it's the job. What you'll do: one role, two hats Hat #1: Wellness Champion (Revitalize Wellness) This is the heart of the community. Every senior living resident has access to your complimentary wellness services. Host the Wellness Center during set hours for drop-in consultations, screenings, blood pressure checks, and follow-ups. Get to know each resident through our six-dimension Wellness Wheel (physical, social, intellectual, emotional, and more) and create personalized wellness plans. Share health education that's practical, engaging, and maybe even fun. Plan and lead senior fitness, educational, and wellness programs, and team up with community organizations and universities to bring fresh ideas to campus. Track wellness trends and program metrics so we keep getting better. Hat #2: Care Management Guide (billable services) When a resident needs a little more support, you're the one who helps them get it. Spot residents whose needs go beyond complimentary wellness and connect them with Care Management services. Complete assessments, develop care plans, and provide ongoing case management. Screen, arrange, and keep an eye on in-home help and additional health or mental health services. Offer education, counseling, and emotional support to residents and families navigating chronic illness and new health challenges. Be the trusted link between residents, families, long-distance caregivers, and the St. Camillus team. Help residents transition smoothly between levels of care, including pre-admission evaluations and discharge planning. Behind the scenes Document wellness screenings and care management visits accurately and on time, following agency policy and state and federal regulations. Practice infection control and take part in emergency preparedness planning.
