Our growing non-medical home care agency is seeking an experienced, independent Registered Nurse (RN) to conduct in-home client intakes, initial functional assessments, and routine quality care oversight in the Eastern Panhandle (Berkeley and Jefferson Counties).This is a PRN / Per-Visit designed to fit completely around your existing hospital, clinic, or personal schedule. You tell us your geographic availability, and we match you with local seniors and veterans needing care setup. There is no hands-on clinical nursing, wound care, or medication administration required.

Responsibilities Conduct In-Home Intakes:

Drive to local client residences in the Martinsburg and Charles Town areas to perform person-centered functional and safety assessments.

Draft Care Directives:

Formulate the initial task plan—the Personal Attendant Log (PAL)—outlining the daily assistance requirements for our non-medical caregiving staff.

Provide Supervisory Oversight:

Perform brief, mandatory routine in-home check-ins (typically every 30 to 60 days) to review client satisfaction, assess caregiver competency, and sign off on active program records.

Maintain Compliance:

Ensure all completed paperwork satisfies the statutory guidelines of the West Virginia Medicaid Aged and Disabled Waiver (ADW) program.

Skills Active License:

An active, completely unrestricted Registered Nurse (RN) license in the State of West Virginia (Required).

Experience:

Minimum of 1-2 years of nursing experience. Background in Home Health, Hospice, Case Management, or Hospital Discharge Planning is a major plus but not required.

Compliance Readiness:

Willingness to undergo mandatory state fingerprinting and background screening via the

WV CARES

portal.

Reliable Transportation:

Must have a reliable vehicle, valid driver's license, and active auto insurance to commute to client homes locally.

Professional Liability:

Must possess or be willing to obtain an independent professional nursing liability insurance policy ($1M/$3M limits prior to starting client visits).
