Seeking a skilled Nurse Practitioner with experience in home health to conduct comprehensive risk assessments for Medicare Advantage plan members. This per diem opportunity centers on evaluating individuals in their homes throughout Berkeley County, WV, covering areas including Martinsburg, Inwood, and Hedgesville. The focus will be on patient-centered annual health assessments and post-discharge visits, each lasting approximately 45 minutes.

Core Responsibilities:

Perform annual home health risk assessments for Medicare Advantage members Review and evaluate patients prior and current medical history, ongoing conditions, and medication regimens Provide targeted education regarding health conditions and medication management Identify care gaps and recommend actionable solutions No requirements to develop treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in West Virginia Current BLS (CPR) certification Board Certification from AANP or

ANCC MSN

degree from an accredited nursing program Minimum of 6 months Nurse Practitioner experience preferred This position offers a per evaluation compensation of $80-$90, complemented by a mileage stipend and pay for administrative time.
