Key Responsibilities:

Conduct thorough annual home health risk assessments for Medicare Advantage plan members in their homes Review prior and current medical history, conditions, and medication use Provide clear education to patients regarding their health conditions and prescribed medications Identify care gaps and make recommendations to improve patient outcomes No responsibility for creating treatment plans or prescribing medications

Qualifications:

Current Nurse Practitioner license in West Virginia Active BLS CPR certification Board Certified (AANP or ANCC) MSN degree from an accredited nursing school At least 6 months of recent Nurse Practitioner experience preferred Nurse Practitioners with home health experience who value independence will thrive in this community-centered position.
