Key Responsibilities:

Conduct in-home health risk assessments, averaging 45 minutes per visit, for members of a Medicare Advantage plan Evaluate patients prior and current medical history, medical conditions, and medication usage Provide educational support around medical conditions and prescribed medications Identify and communicate gaps in care, offering actionable recommendations to ensure continuity and quality of care Assignments will include areas throughout Marion County, such as Fairmont, Mannington, and Pleasant Valley No development of treatment plans or medication prescribing required

Qualifications:

Active Nurse Practitioner license in West Virginia Current CPR certification (BLS) Board certified by AANP or ANCC Master of Science in Nursing (MSN) from an accredited institution Minimum of 6 months current experience as a Nurse Practitioner preferred This role offers competitive compensation, with $80-$90 per evaluation, plus a mileage stipend and coverage for administrative time.
