Key Responsibilities:

Conduct comprehensive annual and post-discharge home health risk assessments for Medicare Advantage members Review and evaluate patients medical backgrounds, ongoing conditions, and medication histories Educate members on their health conditions, medications, and preventative care measures Identify care gaps and provide actionable recommendations to help improve member health outcomes No responsibility for prescribing medications or developing treatment plansfocus solely on assessment and education

Qualifications:

Current Nurse Practitioner license in West Virginia Board certification from AANP or ANCC Valid Basic Life Support (BLS/CPR) certification Master of Science in Nursing from an accredited nursing program At least six months of current experience practicing as a Nurse Practitioner is preferred
