Key Responsibilities:

Perform comprehensive home health risk assessments for Medicare Advantage members Review and document participants medical history, existing conditions, and current medication usage Educate patients on best practices regarding their medical conditions and medications Detect gaps in care, advising on potential resolutions to enhance patient outcomes Focus solely on assessment; no prescribing or development of treatment plans required

Qualifications:

Valid and active Nurse Practitioner license in West Virginia Board Certification through AANP or ANCC Current CPR (BLS) certification MSN degree from an accredited school of nursing At least 6 months of recent experience as a Nurse Practitioner (preferred) In this role, youll earn $80-$90 per completed evaluation and receive a mileage stipend and compensation for administrative time, recognizing the importance of your contribution during and after patient visits.
