Key Responsibilities:

Perform annual health risk assessments for Medicare Advantage members in their homes Review and evaluate patients previous and current medical histories Assess ongoing medical conditions, medications, and therapies Provide education on disease management and medication usage Identify any gaps in care and offer recommendations to facilitate proper patient management No requirement to prescribe medications or develop treatment plans

Qualifications:
