Schedule:

Full-time, Monday through Friday Potential on-call responsibilities Structured daily patient visit expectations

Key Responsibilities:

Deliver in-home skilled nursing care and interventions Complete thorough, timely OASIS assessments and documentation Work closely with physicians and multidisciplinary teams to develop and execute individualized care plans Provide wound care, medication management, disease monitoring, and patient/family education Communicate patient updates proactively to the clinical team

Qualifications:

Active RN license in good standing Minimum one year of recent home health RN experience OASIS documentation experience is required Familiarity with HomeCare HomeBase (strongly preferred) Capable of managing a full schedule with daily visits Reliable transportation and valid drivers license
