Key Responsibilities:

Perform comprehensive annual health risk assessments in patients homes, with each evaluation averaging about 45 minutes Review and assess prior and current medical history, conditions, and medication use Deliver clear patient education regarding medical diagnoses and medication regimens Identify gaps in care and recommend appropriate solutions

Qualifications:

Active Nurse Practitioner license in West Virginia Current CPR certification (BLS) Board certification via AANP or ANCC A minimum of 6 months recent Nurse Practitioner experience preferred Master of Science in Nursing (MSN) from an accredited institution Enjoy earning $80$90 per evaluation, in addition to a mileage stipend and compensation for administrative time.
