Key Responsibilities:

Perform annual and post-discharge home health risk assessments for Medicare Advantage members Thoroughly review prior/current medical history, existing conditions, and medication usage Offer education to patients about their health status and prescribed medications Identify gaps in care and recommend actionable solutions No prescribing or creation of treatment plans required

Qualifications:

Active and unrestricted Nurse Practitioner license in West Virginia Current CPR certification (BLS) Board certification through AANP or ANCC At least 6 months of clinical NP experience (preferred) MSN degree from an accredited nursing program This role offers a competitive rate of $80-$90 per completed evaluation, as well as a mileage stipend and compensation for administrative time.
