Key Responsibilities:

Conduct in-home risk assessments for Medicare Advantage plan members, typically lasting around 45 minutes per evaluation Review and document members medical histories, current health conditions, and medication profiles Provide clear education regarding medical conditions and prescribed medications Identify care gaps and offer actionable recommendations No requirement to develop treatment plans or prescribe medication during these visits

Qualifications:

Active and unrestricted Nurse Practitioner license in West Virginia Board Certification through AANP or ANCC Current CPR certification (BLS) MSN from an accredited nursing program Minimum of 6 months of recent Nurse Practitioner experience preferred
