Schedule:

Full-time commitment, Monday through Friday On-call responsibilities may be required 13-week contract duration with extension possibility

Key Responsibilities:

Deliver in-home skilled nursing care and clinical interventions Complete accurate, timely OASIS assessments and documentation Execute wound care, medication management, disease monitoring, and patient/family education Collaborate with physicians and fellow care team members to ensure high-quality, individualized care plans Communicate patient status updates, changes in condition, and critical information promptly to the clinical team

Qualifications:

Active RN license in good standing At least 1 year of recent home health RN experience required Demonstrated OASIS proficiency (required) Experience with HomeCare HomeBase preferred Strong organizational skills to balance daily patient visits and documentation Dependable transportation and valid drivers license
