Key Responsibilities:

Conduct comprehensive home health risk assessments for Medicare Advantage members, with each evaluation averaging 45 minutes Review and document prior and current medical history, medical conditions, and medication usage Provide patient education regarding medical conditions and prescribed medications Identify gaps in care and make evidence-based recommendations Facilitate a smooth patient experiencenote that diagnosis, prescribing, and treatment planning are not required

Qualifications:

Active Nurse Practitioner license in West Virginia Board Certification via AANP or ANCC Current Basic Life Support (BLS/CPR) certification Graduate of an accredited MSN program Six months or more of recent NP experience preferred In this role, youll earn between $80 and $90 for each completed evaluation, augmented by a mileage stipend and compensation for administrative time.
