Assessments typically take 45 minutes and may be performed as part of annual evaluations or following hospital discharges.

Key Responsibilities:

Complete in-home annual health risk assessments for Medicare Advantage members Review and evaluate patients medical histories, current conditions, and medications Offer education on chronic and acute health concerns as well as prescribed medications Identify care gaps and suggest actionable solutions to improve patient outcomes No requirement to create treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in West Virginia Current BLS/CPR certification Board certification via AANP or ANCC Master of Science in Nursing (MSN) from an accredited program At least six months experience as a Nurse Practitioner preferred This role offers compensation of $80-$90 per completed evaluation, along with a mileage stipend and paid administrative time, providing both flexibility and competitive earning potential.
