WESTOVER, WV
Coverage Areas:
Ohio, Marshall, Wetzel and Tyler Counties Shift :
M-F 8am-4:30pm
On Call/Weekend Requirement :
at least one weekend a month along with a Friday and 4 other weekdays.
Pay Rate:
$49/hr - $50/hr
Roles & Responsibilities:
Conduct skilled nursing assessments in patients' homesComplete OASIS visits, including:

Start of Care (SOC)Resumption of Care (ROC)RecertificationsFollow-up assessmentsDischarge assessmentsEvaluate patient conditions and identify changes that require intervention or care plan modificationsDeliver care in accordance with physician orders and individualized care plansMonitor patient progress and clinical outcomesPerform medication reconciliation and patient educationSupport disease management and promote safe care in the home environmentUtilize Homecare Homebase (HCHB) for patient documentation and care coordination
Minimum Qualifications:
Active and unrestricted Registered Nurse (RN) LicenseExperience with OASIS assessments and documentationExperience using Homecare Homebase (HCHB)Valid driver's licenseReliable transportation and current automobile insurance
Preferred Qualifications:
Previous Home Health Nursing experienceCurrent and up-to-date clinical skills across a variety of patient populations
