Job Description

Job Summary:
Facilitate interactions between patients and their physicians. Provide logistical support for patients needing diagnostic and/or treatment planning. Secure referrals and assist with financial and insurance issues. Provide guidance and coordination of services for patients and families across the continuum of care. Assists the Director of Case Management with daily operations and other responsibilities as needed. Demonstrate and encourage team behavior and exceptional patient/guest experiences. Uphold and promote patient safety and quality.
Education:
High school diploma or GED required; Associate's degree in healthcare administration, business, social services, or a related field preferred.
Experience:
Minimum of two years of experience in a healthcare, medical office, customer service, patient access, care coordination, or similar patient-facing role preferred.
Additional Skills/Abilities:
Demonstrated knowledge of healthcare terminology, hospital services, insurance processes, and referral procedures preferred.
