Advanced Practice Provider (Nurse Practitioner or Physician Assistant) Primary Care | Outpatient | Value-Based Care Practice Primary Care the Way It Was Meant to Be Practiced. We are seeking an experienced and compassionate Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our primary care clinic focused on value-based, relationship-driven care. This role is ideal for an APP who believes great primary care is built on time, trust, prevention, and long-term outcomes--not rushed visits and volume-driven metrics. In our clinic, APPs are trusted primary care providers with their own patient panels, supported by interdisciplinary teams and a care model designed to keep patients healthy, engaged, and out of the hospital.
