  • acute care and emergency department discharge using a patient centered approach and effective communication.
The PSN displays a willingness to explore and acknowledge patient needs, expectations and values. She/he responds to patients needs in a way that are helpful and beyond expectation. The goal is to collaboratively work with patients and the health care team to guide the patient to the right care in the right place at the right time by making a follow up appointment with provider. Position has a Monday

50 an hour Essential Duties and Responsibilities Demonstrates effective communication and problem solving skills. Establishes and maintains daily communication with assigned emergency departments, discharge planners, and hospitalists to learn of GVHC patient activities in those facilities. Enhances access to care for new GVHC patients by making appointment for appropriate follow up care. Communicates with patients/families who have had recent hospital visit to ensure appropriate follow up. Communicates all important information clearly and promptly and in real time following the chain of command. Carries out tasks to medical and support service plans by linking patients with appropriate resources and appropriate referrals to outside agencies. Documents all patient encounters in the patient's medical record. Coordinates care plan adherence activities such as appointment reminders, arranging transportation, etc. with hospital discharge planners. Participates in all departmental in-service training, quality improvement activities, and departmental staff meetings as appropriate. Prepares reports in a timely fashion as directed and may be responsible for performing grant related data entry. Maintains personal knowledge base of available resources, both in-house and externally. Maintains Age specific competency. Other duties and projects as assigned. Physical Demands Must be able to lift up to 40 pounds and push up to 100 pounds (on wheels), hear staff on the phone and those who are served in-person, speak clearly in order to communicate information to clients and staff, have vision that is adequate to read memos, a computer screen, personnel forms and clinical and administrative documents, have high manual dexterity, reach above the shoulder level to work, and must be able to bend, squat and sit, stand, stoop, crouching, reaching, kneeling, twisting/turning. Work Environment The noise level in the work environment is usually quiet to moderate.
Education/Experience Requirements Minimum Qualifications:
Bilingual English/Spanish required Work independently within established protocols with minimum supervision. Travel as required for training, meeting and job duties. Strong Communication/motivational interviewing skills. Work weekends, holidays and evenings. Effectively manage caseload. Excellent knowledge and ability to learn computer programs such as Microsoft Office, EHR, and patient tracking software.
Education/Experience:
High School Diploma or equivalent required. A minimum of 3 years' experience in an ambulatory or impatient care setting as a Medical Assistant (with a Medical Assistant certificate from an accredited program), Referral Clerk, Health Information Representative or Certified Nursing Assistant required. Current American Heart Association Basic Life Support Cardiopulmonary Resuscitation certification.
