The ideal candidate will possess extensive knowledge of hospice care, acute and home health services, and demonstrate exceptional leadership in managing patient-centered care programs. This role offers an opportunity to make a meaningful difference in patients' lives by overseeing clinical operations, ensuring compliance with healthcare regulations, and fostering a collaborative team environment. The Director will be responsible for maintaining excellence in patient care delivery while managing operational and administrative functions within the hospice setting. Duties Provide strategic leadership and oversight of all patient care services, ensuring adherence to hospice standards, Medicare regulations, and state licensing requirements. Supervise nursing staff and interdisciplinary team members, including nurse managers and case managers, fostering professional development and maintaining high performance standards. Develop, implement, and evaluate individualized care plans tailored to each patient's needs, ensuring comprehensive symptom management and quality of life improvements. Monitor patient progress through regular review of medical records, documentation accuracy, and compliance with HIPAA privacy regulations. Oversee shift management to ensure adequate staffing coverage across all hours of operation while maintaining optimal patient monitoring and safety protocols. Manage departmental budgets, resource allocation, and operational efficiencies to support sustainable service delivery. Collaborate with physicians, social workers, chaplains, and other healthcare professionals to coordinate holistic patient care approaches. Ensure staff compliance with medical terminology standards, physiology and anatomy knowledge requirements, and documentation review procedures for accurate case management. Requirements Proven experience in critical care environments such as hospice care, home health, assisted living facilities, or acute care settings. Demonstrated supervisory experience with a strong background in nurse management and team leadership within healthcare organizations. Extensive knowledge of EMR (Electronic Medical Records) systems for documentation review and patient record management. Familiarity with Medicare regulations related to hospice services and reimbursement processes. Strong understanding of medical terminology, physiology, and anatomy relevant to end-of-life care. Experience managing shift schedules effectively to ensure continuous patient monitoring and safety. Ability to develop comprehensive care plans aligned with best practices in hospice and case management principles. Knowledge of HIPAA compliance standards for safeguarding patient information across all documentation activities. Excellent leadership skills with a focus on fostering a collaborative environment that emphasizes quality improvement and compassionate care delivery.
