$34-$35 per hour Join Our Admissions Team! St. Francis Healthcare Center is seeking a dedicated, organized, and compassionate Admissions Coordinator to join our team on a full-time basis. The Admissions Coordinator plays a key role in supporting the admissions process, coordinating referrals, communicating with residents and families, and working closely with clinical and administrative departments to ensure a smooth transition into our facility. If you have strong communication skills, attention to detail, and experience in healthcare admissions or post-acute care, we would love to hear from you! Key Responsibilities Coordinate and facilitate the admission process for new residents. Review and process referrals, medical records, and required admission documentation. Communicate with hospitals, physicians, case managers, residents, families, and referral sources. Obtain and verify insurance and demographic information. Coordinate admissions with nursing, social services, business office, and other departments. Ensure all required paperwork and documentation are completed accurately and timely. Maintain accurate admission records and referral information. Follow up with referral sources regarding pending and prospective admissions. Assist with coordinating transportation and admission arrangements when needed. Provide residents and families with information regarding facility services, policies, and procedures. Help ensure a welcoming and organized admission experience. Maintain confidentiality and comply with HIPAA and facility policies. Support census development and building relationships with referral partners. Perform other duties as assigned. Qualifications Previous experience in healthcare admissions, skilled nursing, post-acute care, hospital case management, or a related healthcare setting preferred. Strong verbal and written communication skills. Excellent organizational and time-management abilities. Ability to multitask and prioritize in a fast-paced environment. Strong attention to detail and follow-through. Professional and compassionate approach when interacting with residents and families. Knowledge of insurance verification and healthcare documentation preferred. Proficiency with computers and electronic medical record systems preferred.
