Job Description

Job Title:
Director of Patient Care Services (Hospice) About the Role A well-established and compassionate hospice care provider is seeking a dedicated Director of Patient Care Services (DPCS) to lead our clinical operations. This leadership position is ideal for a Registered Nurse with a strong background in hospice or palliative care and a passion for ensuring patients receive exceptional, empathetic end-of-life care. As the DPCS, you will oversee the coordination and quality of care provided by our interdisciplinary team, manage clinical staff, ensure compliance with regulatory standards, and drive quality improvement initiatives. This role plays a critical part in supporting not only our patients and their families, but also our devoted clinical team.
Key Responsibilities Clinical Leadership:
Oversee delivery of hospice services in compliance with federal/state regulations and internal standards.
Team Management:
Recruit, train, mentor, and supervise clinical staff including RNs, LVNs, social workers, and home health aides.
Care Coordination:
Ensure development and execution of personalized patient care plans in collaboration with the interdisciplinary team.
Regulatory Compliance:
Maintain compliance with Medicare/Medicaid guidelines and accreditation standards; manage documentation and audits.
Quality Improvement:
Lead initiatives that enhance patient outcomes and overall service quality.
Budget Oversight:
Manage clinical budgets and monitor resource utilization.
Family Support:
Serve as a liaison to patients' families, providing clear communication and emotional support.
Cross-Department Collaboration:
Work closely with social services, spiritual care, and administrative departments to provide comprehensive care.
After-Hours Readiness:
Develop and enforce protocols for emergency and after-hours patient needs.
Qualifications Education:
BSN, MSN or related advanced degree preferred.
Licensure:
Active RN license in
CALIFORNIA
required.
Experience:
Hospice or palliative care, with at least 2 years in a leadership role.
Skills:
Strong leadership and staff development capabilities Deep understanding of hospice regulations and standards Effective communicator with patients, families, and staff Collaborative mindset and critical thinking abilities Physical & Travel Requirements Ability to visit patients in homes, nursing facilities, and hospitals as needed. Comfortable working in varied care environments.
