JOB OPPORTUNITY
We are seeking a dedicated individual to join our Wellness Center team in Sebastopol, CA. The Patient Services Representative is primarily responsible for coordinating administrative workflows, managing patient information in the electronic health record (EHR), and serving as a key liaison between patients, providers, and care teams. This role plays an active role in optimizing clinic flow, supporting care coordination, and enhancing patient engagement through a welcoming, organized, and patient-centered approach. The ideal candidate will serve as a vital link between patients and healthcare providers, ensuring a smooth and positive experience for all patients. Position is a full time, on-site role, Monday-Friday (8:45a-5:15p) located in Sebastopol, California and and may require some evening (12p-8:30p) and rotating Saturday (8:30a-12p) schedules in the future. DUTIES Greet patients and guide them through the check-in and visit process to ensure a positive and supportive experience Actively manage patient flow throughout the visit to support efficiency and patient satisfaction Provide real-time support to patients during wait times by connecting them to services, programs, and resources Schedule appointments, manage provider schedules, and oversee same-day scheduling needs Conduct appointment reminder calls and outreach to reduce no-shows and optimize access Monitor clinic and group schedules Support patient registration, eligibility verification, and enrollment in programs such as Medi-Cal, Sliding Scale, BCAP, and Family PACT Coordinate with Access Coordinators to ensure patients are connected to appropriate coverage and services Introduce available Wellness programs and services to new patients Accurately manage patient information within the EHR, including scanning, filing, and processing incoming documents (faxes, medical records, lab/radiology results) Perform general administrative tasks including copying, scanning, faxing, mail distribution, and supply management Schedule, facilitate, and document Care Team meetings Support clinical data tracking, outcome reporting, and quality improvement initiatives Assist in the adoption and implementation of new workflows, technologies, and systems to improve care and efficiency
EXPERIENCE
Bilingual Spanish/English preferred. Previous experience in an office or medical office setting preferred. Experience in an outpatient or community health clinic setting. Familiarity with medical administrative support functions. Knowledge of medical terminology is essential for effective communication with healthcare professionals. Strong organizational skills and the ability to multitask effectively in a busy environment. Excellent interpersonal skills to interact positively with patients and staff alike. Proficiency in using office software and electronic health record systems is beneficial.
PHYSICAL REQUIREMENTS
Ability to move freely about the clinic/office and carry light loads (up to 25 lbs.) Prolonged standing and /or sitting required. Ability to interface with electronic office equipment for extended periods of time.
