Full Job Description Description:
We are Certified Great Place to work! Position Summary We are seeking a highly organized, customer-focused Admissions Coordinator to join our Skilled Nursing and Long-Term Care team. This is a non-clinical role responsible for managing the admissions process from initial referral through resident admission, ensuring a seamless experience for residents, families, hospitals, and referral partners. The ideal candidate will have strong communication skills, knowledge of post-acute care admissions, and the ability to build relationships while supporting census growth and exceptional customer service. Admissions Coordinators in SNF/LTC settings commonly manage referrals, coordinate admissions, conduct tours, verify payer sources, and serve as a liaison between referral sources, families, and facility leadership. Essential Duties & Responsibilities Serve as the primary point of contact for prospective residents, families, hospitals, and referral sources. Manage incoming referrals and admission inquiries via phone, email, fax, and electronic referral systems. Coordinate the admission process from referral through admission. Verify insurance coverage and payer sources, including Medicare, Medicaid, Managed Care, and private pay. Schedule and conduct facility tours for prospective residents and families. Communicate facility services, amenities, and admission requirements. Collaborate with facility leadership to facilitate admission decisions and ensure appropriate placement. Prepare and maintain admission records, referral tracking logs, and census reports. Develop and maintain positive relationships with hospitals, case managers, discharge planners, physicians, and community partners. Follow up with new residents and families to ensure a positive transition and customer satisfaction. Assist with census development initiatives, community outreach, and marketing activities. Ensure compliance with all facility policies and applicable federal, state, and payer requirements. Requirements:
High school diploma or equivalent required; Associate's or Bachelor's degree preferred. Minimum one (1) year of admissions, healthcare customer service, business development, or related experience preferred. Experience in Skilled Nursing, Long-Term Care, Rehabilitation, Assisted Living, Home Health, or Healthcare Admissions strongly preferred. Knowledge of Medicare, Medicaid, Managed Care, and insurance verification processes preferred. Strong interpersonal, organizational, and problem-solving skills. Excellent verbal and written communication abilities. Proficiency with Microsoft Office and electronic medical record/admissions systems.
