We are currently seeking a dedicated, full-time Intake Coordinator to join our Rocky Hill team. Here's how you will make an impact: Responsible for processing home health referrals in an accurate, timely manner, includes accurately completing data input and client information from referral sources in a variety of manners (phone/fax/email/portals/etc) Ability to learn and retain intake protocols Serve as the liaison between company, referral sources, and patients/caregivers Track and follow up with all patients who are pending starts of care, including face to face visits, paperwork, and physician visit note tracking Understand and effectively communicate the services offered by the agency Attach, scan, and upload referral information and other required patient documents into the agency's electronic medical record system Interact with referral sources and business development team regarding new referrals All other duties as assigned by supervisor 40 hours/week, including weekends. We are happy to offer flexibility with our schedule options. Please inquire within! What qualities and experiences will you bring to our team? Current and active CT Licensed Practical Nurse License in good standing Proficient with Medicare guidelines At least one (1) year of home health experience preferred At least one (1) year of experience in a customer service capacity within a medical setting preferred Proficient typing, using a computer, and multiple software applications (Microsoft office, Google Apps, and electronic medical records) Must be able to document accurately and appropriately Demonstrates patience, flexibility, and a cooperative attitude Able to provide excellent customer service Position requires a majority of interaction over the phone-must be able to communicate clearly and concisely both verbally and in written communications Able to effectively manage concurrent demands and multi-task Familiarity with medical terminology preferred Ability to work well in a team environment Experience in an accredited organization preferred Valid driver's license in good standing and motor vehicle insurance Must have own dependable transportation kept in good working condition Ability to function in a fast-paced, high volume environment At Masters In Home Care, we believe in nurturing our team.
