Full Job Description Outpatient Primary Care | Monday-Friday | No Weekends | Altamonte Springs, FL 32714 Spanish Speaking Required Provenir Healthcare is partnering with a respected outpatient primary care practice in Altamonte Springs, Florida that is seeking a Bilingual Nurse Practitioner (APRN) or Physician Assistant (PA-C) to join its growing team. This opportunity is ideal for an APP who is passionate about senior-focused primary care , enjoys building long-term patient relationships, and wants to work in a supportive, value-based care environment where quality and prevention are prioritized over high patient volume. What Makes This Opportunity Different Full-time employed position 100% outpatient primary care Monday-Friday schedule (8:00 AM-5:00 PM) No evenings, weekends, holidays, or call responsibilities 13-15 patients per day on average Longer appointment times that support personalized, relationship-based care Collaborative team including physicians, nurses, medical assistants, and care coordinators Value-based care model focused on improving patient outcomes and reducing unnecessary hospitalizations For immediate consideration email CV/resume to Your Responsibilities Provide comprehensive primary care for adult and senior Medicare Advantage patients Evaluate, diagnose, and treat acute and chronic medical conditions Focus on preventive care, wellness visits, and chronic disease management Educate patients and families on treatment plans and healthy lifestyle choices Coordinate care with the interdisciplinary healthcare team Maintain accurate and timely clinical documentation Qualifications Graduate of an accredited Nurse Practitioner or Physician Assistant program Active Florida APRN or PA-C license National Board Certification required ( AANP, ANCC, or NCCPA ) Fluent Spanish and English communication skills required 1-2+ years of primary care experience preferred Experience with Medicare Advantage, geriatrics, or value-based care is highly preferred Strong commitment to compassionate, patient-centered care Please TEXT Sierra at 210-591-7125 to set up a time to talk!
